Patient Assesment

You may call on behalf of yourself or a loved one seeking drug or alcohol rehabilitation services from JourneyPure At The River. Please be prepared to supply us with the necessary information needed to complete a patient assessment:

Patient’s full name, birthdate and social security number
Patient’s insurance card
Patient’s drug and alcohol addiction history
Patient’s mental health history

You can pay for your stay at JourneyPure at the River using any of the following methods:

-Health Insurance Coverage

Admission

Once enrolled, you will be given a specific time to arrive at the facility. Our friendly staff will be there to welcome you.

What To Bring:

Patients may bring one bag with them.
Up to 7 days' supply of clothing both casual and workout attire for outdoor activities
Tennis shoes that can get dirty
Toiletries
Prescription medications in the original bottles with prescribing doctor’s information clearly visible. These medications will be administered by our staff.
Cigarettes/Vape
Books, a journal, a drawing pad
Photo ID and insurance cards
Credit or debit card
$100 Cash recommended

What Not To Bring:

Valuables
Electronics
Food or beverages, no powdered drinks
Stuffed animals
Outside bedding or pillows
Aerosol sprays, perfume/cologne, hand sanitizer or mouthwash containing alcohol
Drugs/Alcohol
Weapons of any sort
